Changing Your Doctor

You can change yor doctor at any
time and without having to give a
reason. Take your medical card (which
has your NHS number) to the practice
you have chosen. The doctor is allowed
to refuse to accept you.
If you have lost your medical card
Write to your Family Health Services
Authority giving your name, address and
date of birth. They will send you a new
medical card which you can then take to
the doctor of your choice. The new
practice may accept you without a
medical card if you are able to give
enough details on a special form.
